Two-vehicle crash leaves Leesville woman dead, 5 others injured

LEESVILLE, LA (KPLC) -

A two-vehicle accident occurred Thursday afternoon on La 8 near Hwy 171 in Leesville, La., leaving one woman dead and five others injured.

According to investigators, a 2007 Chevrolet Uplander, driven by 39-year-old Hebert Lazarus of Leesville, was traveling westbound on La 8, when it began to hydroplane. Lazarus lost control and crossed the centerline, causing him to collide with a 2000 Chevrolet Tahoe, driven by 56-year-old Stephen Johannes.

Johannes had three other passengers in the Tahoe: 33-year-old Joey Miller of DeRidder, 50-year-old Lance Mitcham of Leesville, and 20-year-old Justin Stephens of Leesville. All occupants of the Tahoe were transported to a local hospital for treatment of minor injuries.

Lazarus' only passenger, identified as 34-year-old Tina M. Lewis, was pronounced dead at the scene. Both she and Lazarus were wearing their seatbelt. Lazarus was transported to a local hospital for treatment of minor injuries.

Routine toxicology samples were submitted for testing. The crash remains under investigation.
